We're looking for an experienced Care Navigator/Receptionist to join our team. Are you a people-focused problem solver who thrives in a busy, community-centred environment? We're looking for a Care Navigator to become a vital part of our GP practice team, helping patients access the right care, at the right time, with the right clinician or service. Salary is dependent upon experience.
Main duties of the job
As a Care Navigator, you'll be the first point of contact for our patients, whether by phone, online, or in person. You'll use your excellent communication skills, empathy, and initiative to guide patients through their healthcare options, ensuring they receive safe, timely, and appropriate support.

We are a small, friendly 3 partner practice based in Spital, Wirral. We have just under 5000 patients.
Job responsibilities
- Welcoming and supporting patients with warmth, professionalism, and respect
- Using care navigation protocols to direct patients to the most suitable clinician or service
- Managing appointment requests, repeat prescriptions, and general enquiries
- Supporting patients to use digital tools such as online consultations and NHS app services
- Maintaining accurate records and handling confidential information appropriately
- Identifying patients who may benefit from additional support such as social prescribing or community services
- Administration such as handling online consultation requests, scanning etc. using a variety of IT systems
