We are looking for an experienced Diabetes Specialist Nurse who is an Independent Prescriber to join our growing clinical team. This role will support adults living with diabetes through a remote-first, structured support service.
You will help patients understand their glucose patterns, build confidence in self-management, use technology safely and make informed decisions about their diabetes care. You will work within a supportive, consultant-led service alongside colleagues across Spirit Health, GP practices and wider NHS teams.
What you’ll do
You will:
- Review glucose data and wider clinical information.
- Provide clear, personalised diabetes education and advice.
- Support safe medicines optimisation and prescribing within your scope of practice.
- Identify and escalate clinical concerns appropriately.
- Maintain accurate clinical records and communicate clearly with GP practices.
- Contribute to a safe, compassionate and practical service for patients.
About you
You will have:
- Current NMC registration
- Independent prescribing qualification
- Strong diabetes experience, ideally as a Diabetes Specialist Nurse or in a diabetes-focused role in a community setting.
- Confidence supporting people with Type 2 diabetes.
- Experience with insulin management and diabetes medication optimisation.
- Excellent communication skills, particularly when working remotely.
- Sound clinical judgement and strong documentation standards.
- A warm, patient-centred approach.
