Rethink Mental Illness is recruiting a Care Navigator to support individuals transitioning from prison to the community, ensuring continuity of care and effective release planning at HMP Millsike. You will reconnect people with health services, promote wellbeing and contribute to reducing health inequalities through coaching-based interventions.

Working alongside prison staff, healthcare providers and partner agencies, you will deliver person-centred 1:1 and group support, complete screenings.
