Job Description

We are working with a local authority seeking a committed Adult Social Care Assessor to join their front line team

What you will do:

  • Complete assessments and develop care and support plans for adults with a wide range of needs.
  • Review existing support plans to make sure they continue to meet individual goals and promote independence and well being.
  • Work closely with adults, families, carers, and professionals, using a person-centred approach and evidence-based practice.
  • Support people to access services, equipment, and resources - always balancing independence, risk, and safety.
  • Play an active role in safeguarding adults and children, ensuring concerns are identified and raised promptly.
  • Handle sensitive information with empathy, managing situations where individuals may be distressed or displaying challenging behaviour.
  • Contribute to team tasks, provide feedback, and support colleague's learning and development.

What we're looking for:

  • Experience of working with adults, families, or vulnerable groups in a care or support role.
  • A good understanding of adult social care, safeguarding, and risk management.
  • Strong interpersonal and communication skills, with the ability to de-escalate tense situations.
  • Organisational skills to manage a varied caseload and respond to changing priorities.

Hours: 36.5 hours each weekContract: 3-6 months with the expectation of extension.Rate of Pay: £19.00-£24.00
