Make a meaningful difference in the lives of vulnerable young people as they transition to independent living in this rewarding social care role. We are assisting a Local Authority in the appointment of an experienced Leaving Care Specialist (Personal Advisor) to join their Leaving Care Team in Salford! As a Leaving Care Specialist you will be expected to effectively support young people who are \”Looked After\” (in care) or have recently left care in the important component parts that make up their individual lives in order to maximise their life opportunities and help them reach their full potential. Key Responsibilities: To facilitate, directly and indirectly the assessment of need and engaging with the ongoing monitoring and reassessment of need over time. Provide practical, personal and emotional support to children and young people in order to meet assessed needs and promote their health, physical emotional development and wellbeing. Support and advise young people leaving care in key areas including health, benefits, education, training, employment, accommodation/housing and personal wellbeing. Identify and support young people to overcome barriers to learning. Enable and empower young people to make decisions in their lives and to self-advocate and advocate on behalf of young people who do not have the skills to do this. Facilitate the construction of a Pathway Plan, from the assessment which establishes a viable response to established need over time.Requirements: NVQ Level 3 or equivalent. Experience of direct work with children, young people and families. Full UK driving licence.
