We're recruiting an experienced Occupational Therapist to join an integrated Reablement Service within Adult Social Care, on behalf of one of our partnerships in the East London area. You'll work with service users in the community for up to six weeks on tailored rehabilitation programmes, supported by Reablement Officers, to help prevent, reduce, or delay long-term care needs.
Key Responsibilities
- Manage a varied caseload of hospital discharges and community referrals.
- Complete assessments, care plans, and reviews to promote independence.
- Oversee reablement interventions delivered by Reablement Officers.
- Carry out risk-assessed home visits as required.
- Work collaboratively with a strong, supportive multi-disciplinary team.
Key Requirements
- Minimum 2 years' post-qualified experience as an Occupational Therapist.
- Previous experience in case management and physical rehabilitation.
- Confident working in a fast-paced community or reablement setting.
- Band 6 level/equivalent - not suitable for newly qualified or Band 5 OTs.
- London-based, with flexibility for office (2 days/week), community, and remote working.
- Monday to Friday with flexible hours.
Contract Length: 3-6 months with a view to extend to an ongoing contract.
Pay Rate: Β£28-Β£39 per hour (Negotiable DOE)
Working pattern - 37 hours per week, Hybrid and Flexible.
