We are working with a client in the Wirral area to recruit a Leaving Care Personal Advisor to support young people transitioning from care into independent living. You will manage a caseload of young people aged 16-25, supporting them to develop and review Pathway Plans in line with the Leaving Care Act 2000. Working in a young person-centred way, you will help them build the skills, confidence and stability needed for independence.

Key Responsibilities

  • Manage and support a caseload of care leavers
  • Develop and review Pathway Plans
  • Support with housing, benefits, budgeting and life skills
  • Assist with employment, education and training opportunities
  • Promote access to health services and wellbeing support
  • Advocate with external agencies
  • Maintain accurate records and attend review meetings
  • Work collaboratively within the team and with partner agencies

Experience and Skills Required

  • Experience working with young people leaving care, including developing and reviewing Pathway Plans in line with statutory requirements
  • Strong understanding of the Leaving Care Act 2000, safeguarding responsibilities, and the challenges faced by care leavers transitioning to independence
  • Ability to effectively coordinate multi-agency support, advocate for young people, and contribute to planning and review meetings
  • Confident in managing a caseload, maintaining accurate records, and working both independently and collaboratively within a Leaving Care team.

Contract Length: 3-6 months with a view to extend to an ongoing contract.

Pay Rate: £22 per hour

Working pattern: 37 hours per week, onsite.
