We are supporting a client in the Bridlington area who is looking for a dedicated Family Coordinator. This role focuses on supporting individuals and their families, ensuring they receive the right guidance, communication, and care throughout their journey.
Key Responsibilities:
- Providing family support by acting as the main point of contact and offering guidance throughout the support process.
- Coordinating communication between families, professionals, and services to ensure everyone stays informed and aligned.
- Supporting care planning by helping to create, review, and update plans that meet the individual's needs.
- Monitoring progress and reporting any changes, concerns, or updates to the relevant teams.
- Helping resolve issues by identifying challenges and guiding families towards the right support or solutions.
Key Requirements:
- Experience in family support, social care, or a similar community-based role.
- Strong communication and organisational skills.
- Ability to build trust while maintaining professional boundaries.
