We are currently supporting a client in Crewe who are looking for several Leaving Care Personal Advisors to join their Leaving Care Team. You'll support young people aged 16-25 as they transition out of care and move toward independent living. Working within a dedicated Leaving Care Service, you'll provide practical guidance, emotional support, and structured pathway planning to help each young person build stability, confidence, and long-term independence.
Key Responsibilities:
- Develop, review, and monitor Pathway Plans tailored to each young person's needs and goals
- Support young people with housing, education, employment, health, finances, and life skills
- Advocate on behalf of young people with internal teams and external agencies
- Maintain accurate case records and ensure statutory responsibilities are met
- Work collaboratively with social workers, partner organisations, and community services
Key Requirements:
- Experience working with young people, ideally within leaving care, youth work, or social care
- Strong understanding of safeguarding, risk management, and statutory guidance for care leavers
- Ability to build rapport, manage challenging situations, and work independently
- Excellent communication, organisation, and record-keeping skills
- A proactive, supportive, and person-centred approach
- Enhanced DBS and ability to travel within the Crewe area
Contract Length: 3 months initially with the view of an extension.
Pay Rate: £15.84
Working pattern: 37 hours per week
