In this role, you will conduct holistic, person-centred assessments and provide integrated health and social care support to help individuals maintain independence and improve their quality of life., You will be tasked with delivering key strategic objectives:

Conduct holistic, person-centred assessments, support planning, and reviews for individuals and their carers.

Perform statutory duties under the Care Act 2014, Mental Capacity Act, and other relevant legislation.

Assess for and arrange the provision of minor equipment, adaptations, and assistive technology to promote independence.

Collaborate with health professionals to facilitate timely hospital discharges, prevent admissions, and ensure appropriate community support.
The successful candidate will have the following skills / experience:

Experience conducting holistic, person-centred assessments and support planning.

Knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding procedures.

Experience working in a multi-disciplinary health and social care environment.
