Working under the delegated authority of a Registered Nurse, the post holder plays a key role within the multidisciplinary healthcare team, delivering high-quality, individualised, and non-judgemental care to patients within the prison primary care setting. They support the operational delivery of Prison Healthcare services, ensuring care is accessible, safe, and responsive to patient needs.

The role involves undertaking delegated duties to engage, educate, and encourage prisoners to participate in national health screening programmes, including breast, bowel, cervical, abdominal aortic aneurysm, diabetic retinopathy, and sexual health screening. Responsibilities include recording physical health observations and supporting clinical activities such as phlebotomy, seasonal vaccination campaigns under Patient Specific Directions (PSDs), NHS Health Checks, smoking cessation, and sexual health education to promote improved physical, mental, and social wellbeing.

The post holder also supports practice nurses in cervical screening, long-term condition management, and immunisation clinics, working in line with organisational values: Working Together, Integrity, Respect, Recognition, Compassion, and Innovation, and reporting to Band 5 registered practitioners and above.

Main duties of the job

The post holder is responsible for maintaining accurate, contemporaneous patient records and care plans on the electronic patient record system in line with NHS Information Governance, Caldicott, and GDPR standards. Working collaboratively across professional boundaries, they contribute to a therapeutic environment that supports rehabilitation and recovery. Duties include supporting offender management processes, participating in audits and performance data collection, and adhering to information-sharing policies.

They monitor and observe patients, including those at risk of self-harm or suicide, escalating concerns promptly and working compassionately within a demanding environment. The role involves attending meetings, supporting clinical governance and quality improvement initiatives, reporting incidents, and maintaining safety and security procedures.

Under supervision, the post holder delivers high standards of patient care, undertakes clinical observations, supports care planning, provides 1:1 interventions, and responds to patients in crisis. They promote engagement, consent, and personal responsibility for health, work flexibly across service needs, adhere to organisational policies, and ensure efficient, safe use of equipment and resources.

Person Specification

  • NVQ level 3 in Health and Social care or equivalent clinical experience OR NHS Care Certificate or relevant clinical experience
  • Demonstration of experience of working with people with complex health needs, in a health or social care setting.
  • Knowledge of nursing standards gained through qualification or equivalent experience.
  • Knowledge of current NHS Screening Programmes
  • Demonstration of experience (lived or practiced) in delivering health promotion education and support.
  • Understanding and application of confidentiality
  • The ability to complete tasks under delegated responsibility and ability to escalate concerns to senior clinical staff as appropriate
  • Promote and support patients to manage own mental and physical needs
  • Experience of work in a multidisciplinary team setting.
  • Good patient assessment skills - knowledge of signs and symptoms of a range of mental and physical disorders and its effects on individuals.
  • Experience in monitoring and support with the management of long-term conditions
  • Experience of intramuscular delivery of vaccines working from a PSD
  • Experience of smoking cessation education and support
