Working under the delegated authority of a Registered Nurse, the post holder will be a key part of the Multi-Disciplinary healthcare team, providing high standards of individualised, non-judgemental care for patients and supporting the operational requirements of delivering Prison Healthcare across Primary Care, Mental Health, and Substance Misuse services. The role involves supporting the delivery of holistic care packages within our integrated services. The post holder will undertake delegated duties covering the patient journey from reception of prisoners into prison through to their discharge. This includes reception screening, providing care packages, undertaking physical health observations, clinical duties, clinics such as Phlebotomy, ECG, NHS health checks, sexual health, and health promotion to improve mental, physical, and social needs. The post holder will also work as part of the emergency response team as required. The role requires supporting patients with low-level mental health issues and withdrawal from illicit substances, providing support according to care plans. Patient Care Responsibilities Support, implement, and provide high standards of care under the supervision of a qualified nurse/practitioner. Develop professional relationships with service users to promote effective communication and recovery. Ensure consent is obtained at every intervention. Promote patient engagement and personal responsibility for managing their health, providing necessary support. Assist in developing, reviewing, and evaluating care plans. Respond to emergencies using the response radio and undertake relevant interventions as directed. Deliver 1:1 interventions as per care plans. Support patients in crisis, ensuring appropriate referrals and risk management. Use System1 and complete electronic documentation with appropriate read codes.
