Under the direction of an Advanced Nurse Practitioner you will provide flexible clinical care for our older demographic in the community, with an emphasis on care delivered at home.

This is a full time position working 37.5 hours per week.

Duties will include but are not limited to:

Home Visits

  • Conduct comprehensive nursing assessments for older and housebound patients.
  • Monitor andassistin the management of chronic conditions (e.g., diabetes, COPD, heart failure), within your competencies.
  • Provide wound and catheter careassessment andcarewithin your competencies.
  • Identifyearly signs of deterioration and escalate appropriatelyto supporting clinicians.

Nursing/Care Home Case Management

  • Act as a liaison between the GP surgery and care home staff.
  • Undertake reviews of residents health and care plans.
  • Supportand recordend-of-life care planning and advanced care discussions.
  • Ensure compliance with safeguarding and best practice standards.
  • Deliver seasonal vaccinations (e.g., flu, COVID-19) and other immunisationswhererequired.
  • Promote health education and self-care strategies for older patients.

Multidisciplinary Team (MDT) Meetings

  • Attend and contribute to MDT discussions on frailty.
  • Collaborate with social care, community nursing, and allied health professionals.
  • In-House Acute Older Person ClinicsSupport same-day assessments for acutely unwell older patients.

  • Assistthe practice in the meeting of QOF and similar activity-based NHS targets.

You will be fully supported by and work alongside anAdvanced Nurse Practitioner and assigned Visiting GP along with the broadclinical team at the practice comprising of GPs, Clinical Pharmacists, Paramedics,Practice Nurses, Advanced Clinical Practitioner and Health Care Assistants. Itis a permanent role. Full induction and training is given. We offer a generousNHS pension scheme, staff uniform, five weeks annual leave and bank holidayspro rata.

Person Specification

Experience

  • Understanding of the particular requirements of elderly patients within an acute and community setting and the knowledge and skills to assess patients with commonly recognised admission criteria E.g. Falls, Urinary Tract and Lower Respiratory tract infections, Delirium and Dementia, heart failure and COPD/asthma exacerbations.
  • Ability to prioritise individuals for assessment and management according to their health status and needs, referring for specialist assessment, diagnostic tests and programs of support as appropriate.
  • Develop person led single geriatric care plans in partnership with patients and their relatives.
  • Initiate and ensure Advanced Care Planning is undertaken at appropriate stage of disease process/frailty trajectory, ensuring patients goals are met when requiring palliative care or end of life care in line with Local Organisational End of Life Strategy and NICE Clinical Guidelines.

Qualifications

  • GCSE grade A to C in English and Maths
  • Registered Nurse with Nursing and Midwifery Council

  • Driving licence and vehicle are required for this role.
