The Role
We are looking for a compassionate, experienced and motivated Health Coach to support young people who experience health-related barriers to securing, starting and sustaining employment.
You will deliver tailored, person-centred health coaching through one-to-one sessions and small-group interventions, both in person and remotely. Your work will focus on mental health, neurodiversity, anxiety management, confidence building, health and wellbeing, and practical condition-management strategies.
Working as part of a multidisciplinary team, you will provide specialist support before employment, throughout job preparation and during employment where needed, helping young people improve their wellbeing, build confidence and achieve sustainable employment outcomes.
You will also help young people understand and communicate their health-related needs, including developing Health Adjustment Passports where appropriate. Working collaboratively with Mentors and Employer Services colleagues, you will support conversations around reasonable adjustments and workplace support, ensuring young people receive the right help at the right time.
Key Responsibilities
- Deliver structured, person-centred one-to-one health coaching tailored to individual needs, employment goals and support plans.
- Facilitate small-group health and wellbeing sessions, workshops and programme activities.
- Provide flexible support through face-to-face and remote appointments across the Croydon contract area.
- Support young people experiencing mental health challenges, neurodiversity, disabilities, long-term health conditions and other health-related barriers to employment.
- Deliver practical interventions focused on anxiety management, confidence building, resilience and self-management strategies.
- Help young people identify their strengths, needs, triggers and practical strategies to support their transition into and progression within employment.
- Co-produce Health Adjustment Passports where appropriate, helping young people communicate their needs and identify effective workplace adjustments.
- Work with Mentors and Employer Services colleagues to coordinate support, promote reasonable adjustments and improve job retention.
- Agree measurable actions with young people, monitor progress and review outcomes regularly.
- Build relationships with local healthcare providers, community organisations and specialist services to facilitate appropriate referrals and signposting.
- Recognise and escalate safeguarding, wellbeing and risk concerns in line with organisational procedures.
- Participate in case reviews, supervision, reflective practice, mandatory training and continuing professional development.
- Manage an individual caseload effectively, prioritising appointments and responding flexibly to changing support needs.
- Comply with safeguarding, data protection, equality, diversity, inclusion, health and safety and quality assurance requirements.
Skills and Experience
- A recognised professional qualification in counselling, mental health, psychology, nursing, occupational therapy, allied health, health coaching or another relevant discipline.
- BACP registration/accreditation or an equivalent recognised clinical qualification appropriate to the delivery of specialist mental health support.
- Experience supporting people experiencing mental health challenges, neurodiversity, disability, long-term health conditions or other complex barriers.
- Experience delivering structured one-to-one interventions and facilitating group sessions or workshops.
- Strong coaching, active-listening and relationship-building skills, with the ability to engage young people in a supportive, inclusive and non-judgemental way.
- The ability to deliver practical anxiety-management, confidence-building, resilience and condition-management interventions within professional competence.
- Knowledge of safeguarding, risk management, consent, confidentiality and professional boundaries.
- An understanding of person-centred, strengths-based practice and the Social Model of Disability.
- Excellent communication, case-recording and report-writing skills, with the ability to maintain accurate records and demonstrate progress against agreed outcomes.
- Strong organisational skills, including the ability to prioritise effectively, manage a caseload independently and meet contractual requirements.
- The ability to work collaboratively with colleagues, employers, healthcare professionals and community partners.
