Job Description

Position: Health and Employment Coach

Location: North and West London

Salary: £32,500 – £36,300

Type: Permanent

The Role

We are recruiting a number of Health and Employment Coach to provide highly effective support to individuals and guide them on their journey to returning, remaining or finding new employment while also supporting them to improve their health and wellbeing. The Health and Employment Coach will also be instrumental in supporting participants to manage their Musculoskeletal (MSK) related barriers by working in collaboration with our team of specialists in a community hub environment.

This contract is currently commissioner for 12 months and being reviewed regularly. We are considering all options for recruited staff including retention incentives to reward those who join us.

These roles are based across North and West London, including Barnet, Brent., Ealing, H&F, Harrow, Hillingdon, Hounslow, K&C, Westminster, Camden, Islington and Haringey

Main Duties and Responsibilities

o Manage a caseload of participants with MSK conditions (up to six months), providing advice and guidance to help them return to, remain in, or secure employment.

o Complete NHS-accredited Health Coach training before working with participants.

o Attend community appointment events across London boroughs to generate referrals and build caseload.

o Work from designated community hub locations up to five days a week.

o Meet participants to identify barriers, set goals, and co-produce action plans using the Dialog+ approach.

o Collaborate with the Multidisciplinary Clinical Team (e.g. physio, pain management, therapy, nutrition) to ensure seamless support.

o Assess wider work-related needs and refer to partners for welfare, mental, or physical health support.

o Identify and arrange appropriate provision for participants with significant mental/physical health barriers.

o Advocate with employers to resolve complex workplace issues and support job retention.

o Conduct holistic Dialog+ assessments, promote wellbeing and healthy lifestyle changes.

o Build and maintain strong partnerships with commissioning organisations, referral partners (e.g. social prescribers, MSK teams, GPs, community providers), and external agencies (e.g. colleges, training providers) to grow referrals and opportunities.

o Develop relationships with employers to promote the service, identify job opportunities, and implement workplace strategies that support employees with health needs.

o Connect with wider employment support services to enable smooth handovers when alternative support is required.

Skills & Experience

o Demonstrable experience of successfully working to targets

o Relevant experience of handling complex cases, provision of support to colleagues and/or subordinate employees guiding on best practice and coaching others to achieve excellence in their roles

o Experience providing case management support to a caseload of participants with health and/or employment related barriers

o Understanding of the practical impact of common health conditions on an individual’s ability to work

o Ability to manage a caseload of individuals with different needs and goals

o Practical experience in accessing external support services and well developed networking skills
