We are seeking two experienced and passionate Social Workers to join our Transfer of Care (TOC) Service, supporting adults with care and support needs to achieve safe, timely, and effective hospital discharges.
About The Role
This is an exciting opportunity to work within a fast-paced, multidisciplinary environment, helping adults move smoothly from hospital or rehabilitation settings back into the community or onward care.
Key Responsibilities Include:
- Supporting safe and timely discharges from acute hospitals and rehab settings
- Promoting independence, wellbeing, and person-centred outcomes
- Undertaking strengths-based assessments and care planning
- Managing complex cases involving physical disabilities, sensory impairments, dementia, and complex health needs
- Working collaboratively with health professionals, housing services, and voluntary sector partners
About You
You will be a registered Social Worker with a strong foundation in adult social care and/or health services. You are calm under pressure, reflective in your practice, and committed to person-centred care.
You Will Bring:
- An excellent understanding of safeguarding procedures and best practice
- Up-to-date knowledge of adult social care legislation, including the Care Act and Discharge to Assess (D2A) pathways
- Experience of strengths-based assessments and outcome-focused care planning
- Confidence working within multidisciplinary teams
- The ability to manage complex cases and competing priorities
- A strong commitment to equality, diversity, and inclusive practice
- Professional curiosity and dedication to maintaining high standards of social work practice
This post requires a satisfactory Disclosure and Barring Service (DBS) check at Enhanced - Adults and Children (Barred) level.
