Southwark Council is seeking an experienced Social Worker to join the Transfer of Care (TOC) Service at St Thomas' Hospital. You will help adults with care needs achieve safe, timely discharges and coordinate transitions from hospital to community or ongoing care.
Working in a fast-paced, multidisciplinary team, you will promote independence, well-being and person-centred care, collaborating with Health, Housing and voluntary sector colleagues.
