Southwark Council is seeking a Reablement Officer to work in the community and residents’ homes, helping people regain independence and practical living skills. You will create personalised reablement plans, monitor progress, and coordinate with housing officers, health professionals and community services to secure positive outcomes.
You will join a supportive team within Housing Needs and Support, focused on prevention, tenancy sustainment and effective move-on support, with 36 hours per week.
