Clinical Pharmacy in General Practice is part of an exciting transformation programme, developing a new model of care across Southport and Formby PCN. This approach aligns with national priorities, including the Five Year Forward View and GP Forward View, ensuring pharmacy professionals play a key role in helping patients stay safe, well, and out of hospital, while reducing pressures on general practice.
This is a full-time, 11-month fixed-term position (maternity cover) based at Southport & Formby Health. Part-time applications will also be considered.
Main duties of the job include:
- Working across PCN practices to support clinical pharmacists by undertaking clinical reviews of patient records, identifying and referring or flagging areas of concern.
- Counselling patients, reviewing and accurately recording medications, and participating in audits, with a particular focus on patients prescribed high-risk drugs.
This work is central to improving the quality of care and patient outcomes. The role is pivotal in enhancing both care quality and operational efficiency, and therefore requires a motivated individual with a passion for delivering excellent service within general practice.
