Southgates and The Woottons Surgeries are based in King's Lynn and have a long and proud history of excellent patient care. Our patient population stands at over 22,000. In our latest CQC full inspection of our services, we received an overall rating of 'good'. We are looking for an experienced Advanced Nurse Practitioner (ANP) / Acute Frail Elderly Outreach Matron to join our friendly and professional team in serving our patients. The successful candidate will be an experienced, motivated and enthusiastic ANP/Matron. The candidate must have the Prescribing qualification for this role. The post is for full-time hours (or up to full-time hours), working 3 days per week as an ANP and 2 days per week at a Matron.
Main duties of the job:
- Responsible for the clinical assessment, investigation, diagnosis and treatment and/or referral of patients within urgent care services, identifying complex and specialist needs and developing, implementing and evaluating programmes/plans of care.
- Working as an ANP/Matron with a high degree of autonomy, managing undifferentiated and undiagnosed minor injuries and minor illness, using advanced clinical skills within own sphere of competence.
- Providing direct consultations which, depending upon competency and area of practice, may be via telephone or face to face.

Minimum qualifications and experience required for this post:
- Recognised professional degree or equivalent.
- Postgraduate study at Masters level.
- Registered with appropriate professional regulatory body.
- Evidence of relevant post registration experience.
- Independent Prescribing or evidence of working to PGDs with advanced knowledge (AHPs).
- Evidence of continuing professional development.
- Excellent written and verbal communication skills.
- Knowledge of primary and urgent care issues.

The post holder will take a lead role in the advanced clinical assessment, investigation, diagnosis, treatment and/or referral/discharge of diverse and undifferentiated and undiagnosed problems, in accordance with professional standards and codes of conduct, national best practice and locally and nationally agreed standards.
The Emergency Care Practitioner is expected to:
- Produce, develop and maintain a Personal Development Plan.
- Understand why feedback from others is important in helping to develop and improve the way you work.
- Participate in clinical supervision as a means to improving the quality of care to patients.
- Demonstrate professional conduct as defined by your professional code of conduct preserving patient rights and promoting mutual respect amongst colleagues.
Person Specification:
- Evidence of recent and ongoing personal development including extensive post registration experience in relevant settings.
- Able to demonstrate a working knowledge and experience of the governance agenda locally.
- Experience of education and training issues/requirements for undergraduate and postgraduate members of professions.
- Experience of working with the Frail/Elderly.
