Southern Counties Caring is a dynamic fast developing care organisation specialising in live-in care; home and domiciliary care; night support and supported living services. As a Senior Carer/Assessor, you will be required to carry out comprehensive strength based assessments to inform and enable our care teams to manage their day to day activities effectively and to be in tune with the clients' needs and wants at all times. The essence of this role is to not only enhance the individual's ability to enjoy independent living but, also to safeguard against medical, emotional and other constraints that may impinge on their quality of life. You will be a key player in ensuring that Southern Counties Caring always maintains excellent standards of service.

Required Skills & Capabilities:

  • 2 + years experience or a similar role
  • Strong assessment, observation & decision making
  • Excellent communication & interpersonal abilities
  • Ability to work under pressure
  • Good organizational & IT proficiency
  • Ability to handle queries & concerns professionally & efficiently

Qualifications:

  • NVQ Level 3 or above in Health & Social Care or equivalent
  • Training in safeguarding & risk assessment
  • Medication administration
  • First Aid certification
  • Full UK driving license

Assessment & Care Planning:

  • Conduct comprehensive assessments of individuals' physical; emotional & social care needs
  • Develop & implement person-centered care & support plans
  • Carry out risk assessments and identify safeguarding concerns
  • Review and update care plans regularly to reflect changing needs
  • Maintain accurate care records and equipment servicing records
  • Report any changes or concerns to management or healthcare professionals

Leadership & Supervision:

  • Lead, supervise and support carers to ensure care plans are meticulously followed
  • Provide induction; training; shadowing and guidance to new staff
  • Perform regular spot checks
  • Complete medication audits
  • Update the staff training matrix

Compliance, Monitoring & Review:

  • Ensure all assessments and care plans comply with statutory and organizational standards
  • Follow safeguarding, confidentiality and data protection protocols
  • Adhere to infection control procedures
  • Ensure maintenance of a safe and clean environment
  • Identify and report safeguarding concerns

Client Support & Coordination of Care:

  • Build positive relationships with clients and their families
  • Liaise with families, nurses and other healthcare professionals
  • Provide guidance to clients on suitable care services and support options
  • Support and promote clients' independence, dignity and person-centered care
  • Support clients in accessing community resources and services
  • Ensure continuity of care across services
