We are looking for enthusiastic individuals to join Southend Care's Community Team, working with service users both at their homes and in the community. Flexibility will be important and you will be expected to work across different environments, as well as being expected to join team meetings when required. You will have your own workload assigned to you on the day and will work collaboratively with other professionals for handovers as well as having an in-team ‘buddy’. This is a great rewarding opportunity to support an amazing team and help customers to remain as independent as possible in their own home, whilst having the flexibility to choose the shift you would like to work. The shifts are 6 hours, and can be worked 7am - 2pm, 4 - 11pm 7 days a week.

Key Responsibilities:

  • To be a member of Southend Care's Community team providing a short-term high quality Complex Intervention Service in a community environment in accordance with current legislation, care standards and policies and processes.
  • Enabling safe and timely discharge from hospital along with the prevention of hospital or inpatient/residential admission.
  • To provide support for customers in their own homes, appropriate to the individual needs specified in their care plan and in accordance with current legislation, care standards and policies and processes.
  • To work in a way that promotes the independence, privacy, dignity and choice for customers and to support their social, cultural and religious needs.

Due to the nature of the role being community based, you will be required to hold a full UK Drivers Licence with access to your own vehicle.
