We are seeking an enthusiastic individual to join our friendly community care team as a Field Care Supervisor. As a Field Care Supervisor, you will provide support and supervision to our team of Community Carers. Our Field Care Supervisors currently work 7:00am – 15:30pm and 14:30pm – 23:00pm, 365 days a year, including Bank Holidays and Weekends. You will also be required to be part of an on-call rota.
The Role
As a Field Care Supervisor, you will contribute to the delivery of an efficient and effective community service. Working as part of a team, you will be responsible for the assessment and planning of care provided to our service users.
Day to Day Responsibilities
- To provide effective supervision and support to Community Carers, through supervision of caseloads and managing issues that arise.
- Lead in developing and ensuring the ongoing review of care and support for patients through liaison with community carers, key workers and other appropriate professionals.
- To provide community support and risk assessments for patients in accordance with the medication policy.
- Support patients' health and wellbeing in accordance with the care plan including Reablement, rehab – drugs and alcohol, domiciliary care, SEDS and Mental Health.
- To undertake complex casework assessment visits where necessary.
- To become a champion in a field of service that benefits staff and patients including cascading the information to your team.
- Provide assistance, using appropriate moving and handling techniques or equipment, as necessary, to meet the assessed needs of each individual within the agreed care plan.
- To support a safe and timely discharge with our Enhanced Discharge Service.
- To undertake and manage our POLR customers (Provider of Last Resort) and work in partnership with the allocated Social Worker.
Knowledge, Experience, Skills and Abilities
- A minimum of 2 years’ experience working with adults in a social care setting or community-based.
- NVQ Level 3 in Health and Social Care - or working towards.
- Experience of multi-professional working.
- Knowledge & understanding of rehabilitation process & its role in intermediate care.
- Knowledge of adult protection procedures.
- Good communication skills with the ability to deal with sensitive issues.
- Must hold a full driving licence and have access to a car (essential).
- Flexible to cover the business need.
