Southcare provide essential services to older people who need help so they can stay living independently at home. That means delivering aged care services based on individual needs. We have an in-house clinical team responsible for delivering high-quality, person-centred clinical care to our participants in their own homes. This is a Permanent Full-time role Monday - Friday working in the community. Working hours are between 7.30am and 3.30pm or 8am - 4pm. Join a dedicated team who provide clinical services, conduct comprehensive nursing assessments and ongoing clinical reviews. You will develop, implement and evaluate individual care plans; administer medications and complex clinical interventions in accordance with legislation and scope of practice. You will deliver a range of services and focus on building strong, meaningful relationships with our participants, empowering them to remain independent at home and stay connected.

Skills and Experience:
- A genuine interest in aged care and working in the community and who are committed to making a difference to the lives of older Australians
- Current Registration as Registered Nurse with AHPRA
- Minimum 2 years post graduate clinical experience
- Required to possess a broad range of clinical skills and competencies including but not limited to, the areas of medication management, continence assessments, wound management, and chronic disease awareness & preventative health care
- Strong clinical assessment, decision‑making and documentation skills
- Excellent interpersonal and communication skills
- Ability to work autonomously and manage time effectively
- Current Drivers Licence
- Demonstrated understanding of person-centred care and a willingness to work within Southcare’s Purpose & Values
- National Police Clearance in line with the requirements of the Aged Care Act
