Would you like the opportunity to work in a friendly, supportive, well-established fully integrated, Adult Community Mental Health Team? We offer input to adults with complex and diverse mental health needs. We are dedicated to developing trauma-informed principles of care in conjunction with the Trauma Informed Personality Disorder pathway. This post is based within the Wellbeing team.
The Wellbeing team provides a clinic-based service for medication administration and monitoring; treatment review clinics providing an annual physical health review; side-effect assessment and treatment information/education. We are looking for an enthusiastic individual who is flexible and creative in response to service and patient needs and interested in positively influencing the development of efficient systems. The role involves various responsibilities, utilising different models of working. You will work with a range of professionals including Advanced Nurse Practitioners, Psychiatrists and Pharmacists.
There are considerable professional and clinical supervision opportunities. We also strongly encourage the development of clinical interests through CPD. You will have access to high quality clinical supervision within the team and will work closely with other professionals in the multi-disciplinary team.
You will carry a caseload of service users who you will offer individual interventions to.
You will be responsible for carrying out full comprehensive assessments/risk assessments of each service user with a support package that is tailored to each individual; working with service users to find the best plan of care for their needs; helping service users stay well and working towards and maintaining recovery. You will work closely with the Core Team, Psychiatry, pharmacy GPs, physical healthcare providers, and partner agencies.
