Overview
The post holder will be responsible for the assessment of clients with Learning Disabilities in any setting (i.e. home, acute settings, residential settings) to determine eligibility to receive Fully Funded NHS Continuing Healthcare (CHC) or Funded Nursing Care (FNC). The post holder will act as senior nurse assessor/co-ordinator of a Multidisciplinary Team to establish eligibility and, with the client and their family, identify a suitable care package. They will ensure the review process of all CHC/FNC is undertaken in accordance with the National Framework Guidance for Continuing Healthcare (revised 2018). The post holder will ensure that assessments and reviews are carried out in a timely manner according to the local KPIs in order to meet the national performance targets. The post holder will form part of the senior nurse assessor team for the CHC team, ensuring that clients receive high quality care and provide specialist advice regarding Learning Disabilities to the CHC team. The post holder will also lead on ensuring all clients with a Learning Disability who have care delivered through a Personal Health Budget have appropriate care and support plans in place and will support in the management of the PHB, resolving issues as they arise. The post holder will support the Continuing Healthcare team to ensure that the CHC and FNC work-stream is planned and managed effectively, taking a lead in assisting in their successful delivery.
Responsibilities
- Assess clients with Learning Disabilities to determine eligibility for CHC or FNC.
- Act as senior nurse assessor/co-ordinator of a Multidisciplinary Team to establish eligibility and identify a suitable care package with the client and their family.
- Ensure the review process for CHC and FNC follows the National Framework Guidance for Continuing Healthcare (revised 2018).
- Ensure assessments and reviews are carried out in a timely manner in line with local KPIs and national targets.
- Be part of the senior nurse assessor team for the CHC team, providing specialist advice regarding Learning Disabilities to the CHC team.
- Lead on ensuring all LD clients with care delivered through a Personal Health Budget have appropriate care and support plans and support the management of the PHB, resolving issues as they arise.
- Support the CHC and FNC work-streams to be planned and managed effectively, taking a lead in their successful delivery.
- Establish and maintain communication networks with care home staff, social services, community staff and hospital MDT to deliver CHC assessments and complex case management.
- Maintain effective communication on issues in complex situations arising from CHC criteria, processes and outcomes.
- Prioritise workload and team activity in response to service demands and policy changes; balance clinical and administrative activities.
