This is an exciting opportunity to join a dynamic team in one of our acute inpatient wards as a Staff Nurse at the newly developed Springfield Hospital in South West London. As a Staff Nurse, you will work collaboratively with service users and families, enhancing your skills within the acute care setting and working alongside our multidisciplinary team (MDT) to formulate treatment plans. You will conduct daily risk and mental state assessments of patients with a variety of mental health diagnoses.
Responsibilities
- Work autonomously within the parameters of the role, leading on implementing the delivery of quality nursing care and contributing to physical, psychological, social, recreational and spiritual care in a recovery-focused way through therapeutic engagement and activities, and addressing any issues that arise.
- Lead on and undertake activities to ensure that the fundamentals of care are met: nutrition, hydration, hygiene, comfort, emotional and social support.
- Co‑ordinate and evaluate collaborative patient‑centred care; obtain feedback and improve plans with the MDT.
- Provide accurate information about care in an accessible format to patients and their relatives/carers.
- Liaise and work positively with members of the MDT and partner organisations.
- Administer and monitor medication side‑effects; monitor service user compliance.
- Ensure safety: safe custody of medicines, sharps, clinical equipment and report all incidents following trust policy.
- Develop the management of care needs through initial data collection, monitoring of individual service user/patient progress, feedback and discussion, producing reports and in‑care reviews, devising a plan of care and interventions supervised by a Registered Nurse.
- Lead and direct junior staff in providing hands‑on care, including undertaking 1‑1 engagement with service users in an honest, open and non‑judgmental way.
- Recognise and value service users as individuals, acknowledging the importance of maintaining respect and dignity at all times and ensuring care respects equality and diversity.
- Develop care plans that reflect service user needs and relevant patient care documentation in partnership with service users, wider MDT and relatives/significant others.
- Ensure monitoring and recording of clinical observations (physical observations and vital signs) and treating or escalating any signs of deterioration appropriately and providing health promotion advice.
- Implement the principles of recovery: encouraging and supporting service users to engage with healthy lifestyles, facilitating psychosocial activities and, where appropriate, contributing to psychological interventions.
- Escort service users, as required, in line with Trust policies.
- Facilitate admission and discharge of service users, ensuring needs of the patient and service, including relevant legislative or required documentation, are completed.
- Advocate for service users’ needs and rights within Trust policy and empower service users to have optimal responsibility for their individual programme of care.
Qualifications
- Band 5 nursing role open only to staff with nursing registration obtained through a qualification recognised by the Nursing and Midwifery Council (NMC).
- Experience in acute mental health care preferred.
- Demonstrated ability to coordinate and deliver person‑centred care with a multidisciplinary team.
