An exciting opportunity has arisen for a Recovery Support Worker to join the fast growing, dynamic Community Team. This is a new exciting role, following the development of the Intensive and Assertive Model of Care which seeks to ensure collaborative work with individuals at high risk who need intensive support. You will play a key role in delivering recovery-focused mental health care and providing support to individuals experiencing serious mental illness and their families within the community. This position involves addressing diverse needs and facilitating their journey to recovery.

Key Responsibilities
- Fostering collaborative relationships with individuals experiencing the most challenges in our community and their families.
- Promoting independent living, supporting access to community resources, health services, and welfare benefits.
- Helping individuals develop daily living skills and self‑management strategies.
- Contributing to assessments, care planning, and reviews while providing both practical and emotional support throughout the recovery journey.
- Helping individuals achieve their goals through brief interventions and low‑level psychosocial support tailored to their needs and abilities.
- Engaging with families, carers, and community resources, identifying early signs of relapse, and ensuring timely communication with clinical teams.
- Maintaining accurate record‑keeping and up‑to‑date knowledge of the client group and services.
- Monitoring wellbeing, identifying early signs of relapse, and maintaining accurate records in line with Trust policies.
- Contributing to safeguarding, risk management, and emergency support.
- Fostering professional relationships and advocating for service user needs across services.
- Working collaboratively with mental health professionals and voluntary sector partners to promote independence, self‑advocacy, and social inclusion.
In addition, responsibilities include:
- Working autonomously within the parameters of the role leading on implementing the delivery of quality nursing care.
- Contributing to the physical, psychological, social, recreational and spiritual care in a recovery‑focused way through therapeutic engagement and activities.
- Leading on and undertaking activities to ensure that the fundamentals of care are met - nutrition, hydration, hygiene, comfort, emotional and social support.
- Supporting the patients management of care needs through initial data collection, monitoring of individual service user/patient progress, feedback and discussion, producing reports and in‑care reviews, and devising a plan of care and interventions supervised by a Registered Nurse.
- In a junior leadership role, leading on and directing junior staff in providing hands‑on care to service users.
- Undertaking 1‑1 engagement with service users in an honest, open and non‑judgmental way.
- Recognising and valuing service users as individuals, acknowledging the importance of always maintaining the service user’s respect and dignity.
- Contributing to the development of care plans that reflect service user needs and relevant patient care documentation.
- Escalating any concerns to relevant Health Care Professional.
- Monitoring and recording service user clinical observations, such as physical observations and vital signs.
- Promoting healthy lifestyles and giving health promotion advice and support.
- Implementing the principles of ‘recovery’ by encouraging and supporting service users to engage with healthy lifestyles.
- Where appropriate, contributing to psychological interventions under the guidance of a registered professional.
- Escorting service users, as required, in line with Trust policies.
- Advocating for service users’ needs and rights within Trust policy.
- Encouraging and empowering service users to have an optimum level of responsibility for their individual programme of care.
- Supporting the registered nurse to coordinate and prioritise activities within the clinical area.
