We are looking for a highly motivated qualified Mental Health and Wellbeing Practitioner to join our service. Sutton adult mental health services are comprised of two multidisciplinary mental health teams within an Integrated Recovery Hub which is closely linked to Voluntary, Community and Social Enterprises in the borough. You will also work to with other MDT members including Psychologists, Psychiatrists, CPNs, Mental Health & Wellbeing Practitioners, Recovery & Support Workers and Specialist Employment Advisors in addition to service user stakeholders to understand any potential barriers to accessing the psychological therapies waiting list.

Supervision of psychologically informed interventions will be from a qualified clinical or counselling psychologist. You will receive case management supervision from another qualified mental health professional in the team.

You will work alongside service users, families, carers and multi-disciplinary team members to co-ordinate care, supporting collaborative decision-making about care and treatment.

You will deliver a set of wellbeing-focused psychologically-informed interventions, aligned to cognitive-behavioural principles, based on the best evidence available, that address problems often experienced by people with severe mental health problems.

Responsibilities

  1. Carry out relevant risk assessments and risk management with the multi-disciplinary team
  2. Engage calmly and with sensitivity and empathy to support service users in highly distressing or emotional circumstances.
  3. Engage with and gather information from service users, relatives and patient records to develop a formulation.
  4. Work in collaboration with the individual service user and their family and carers as appropriate to develop care plans that are focused on strengths and are outcome based.
  5. Attend multi-disciplinary reviews and act as a care co-ordinator for a caseload of service users in order to support and monitor progress during the course of multi-disciplinary interventions.
  6. Set collaborative goals for intervention with service users
  7. Deliver specified wellbeing-focused psychologically-informed interventions, in line with best available evidence, under close supervision from a clinical psychologist or CBT therapist including:
    1. Behavioural activation and graded exposure
    2. Teaching problem-solving skills
    3. Improving sleep
    4. Recognising and managing emotions
    5. Guided self-help for bulimia and binge-eating
    6. Building confidence
    7. Medication support
  8. Be responsive to service users’ needs and choices; and uphold their right to be treated with dignity and respect.
  9. Include carers and families in line with the service user’s wishes.
  10. Liaise with other health and care providers, including third sector agencies and primary care, to ensure continuity of care for service users.
  11. Communicate information concerning the assessment, formulation and care plans of service users in a skilled and sensitive manner in order to promote effective multi-disciplinary working and therapeutic outcomes for clients.
  12. Develop collaborative plans for relapse prevention
  13. Deal with endings appropriately and safely with service users, families and carers.
