We are looking to recruit a highly motivated and enthusiastic CPN/Care Coordinator to continue the excellent work in our well‐established team based in the Elizabeth Newton Building, Springfield Hospital, Tooting, London.
Wandsworth Intervention in Psychosis Service offers intensive support to clients who have experienced a first episode of psychosis and are between the ages of 18–65 years. Evidence indicates that interventions at this crucial time can have a beneficial impact on the client and their family in many aspects of their lives. Using NICE‐recommended interventions, including CBT approaches, family work, and intensive outreach, the service offers a positive and recovery‐focused approach.
The successful candidate will have past experience working as a care coordinator in the community and possess robust skills in all aspects of this role. This post requires working autonomously with service users and their families, holding a caseload of approximately 18–20 clients (pro‐rata). The care coordinator is responsible for planning, implementing, recording, and reviewing a comprehensive package of care in conjunction with the multi‐disciplinary team.
Responsibilities
Work autonomously with service users and families, managing a caseload of 18–20 clients (pro‐rata).
Plan, implement, record and review comprehensive care packages with the multi‐disciplinary team.
Ensure fundamentals of care are met: nutrition, hydration, hygiene, comfort, emotional and social support.
Co‐ordinate and evaluate collaborative patient‐centred care; obtain feedback and improve plans with the MDT.
Provide accurate, accessible information about care to patients and their relatives/carers.
Maintain positive liaison with the multi‐disciplinary team and partner organisations.
Administer and monitor medication, including side‐effects and user compliance.
Safeguard medicines, sharps, and clinical equipment; report all incidents in line with trust policy.
Lead, support, and teach junior staff on risk assessment and risk management, applying evidence, clinical judgement and knowledge of the service user\’s context.
Qualifications
Past experience as a care coordinator in the community.
Robust skills in all aspects of care coordination.
Training in psychosocial interventions, including family interventions, is preferred.
