South Waveney Alliance Primary Care Network is seeking an experienced Community Nurse to deliver a high standard of clinical care. We have a well-established practice team including GPs, Urgent Care Practitioners, ANPs, Practice Nurses, Mental Health Practitioners, Enhanced Recovery Workers, HCAs, Physios, Pharmacists, Pharmacy Technicians and Care Coordinators, along with our administrative staff. The practice is committed to innovation, development and providing excellent patient care.

The objective of the Community Nurse is to deliver and develop excellent patient care and support the wider practice teams within the PCN.

Bungay Medical Practice and Sole Bay Health Centre provides the highest quality healthcare to patients. We are part of the SWAN primary care network sharing clinicians and resources and working collaboratively to deliver best care for our patients. Our doctors, large nursing team and all of our staff are dedicated to offering a professional service.

Main duties of the job

  • Clinical Care and Case Management: Undertake comprehensive assessments of patients with complex medical, social, and psychological needs; develop, implement, and regularly review personalized care plans; manage a caseload of vulnerable, frail, elderly, and housebound patients; provide clinical interventions, monitoring, and treatment within the patient's home and community settings; identify early signs of deterioration and implement appropriate interventions; support patients with long‑term conditions such as COPD, heart failure, diabetes, dementia, and other chronic illnesses; undertake urgent assessments to prevent unnecessary hospital admissions where appropriate.
  • Non-Medical Prescribing: Prescribe medications safely and effectively within professional competence and prescribing guidelines; conduct medication reviews and optimize medicines management; monitor treatment effectiveness and potential side effects; promote safe prescribing practices and adherence to local and national policies.
  • Care Coordination and Multi-Disciplinary Working: Act as a central point of contact for patients, carers, families, and healthcare professionals; liaise closely with GPs, district nurses, community nurses, social services, therapists, mental health teams, palliative care teams, hospitals, and voluntary services; coordinate and facilitate multidisciplinary team meetings; ensure continuity of care through effective communication and information sharing; advocate for patients and support access to appropriate services.
  • Patient and Family Support: Provide emotional, psychological, and practical support to patients and their families; support patients in making informed decisions regarding their care and treatment; assist carers by providing education, advice, and signposting to relevant support services; deliver health promotion, disease prevention, and self‑management education.
  • Safeguarding and Risk Management: Identify safeguarding concerns relating to vulnerable adults and take appropriate action in accordance with safeguarding policies; conduct risk assessments and implement measures to minimise risk; maintain accurate, contemporaneous clinical records in line with professional and legal requirements.

Person Specification

  • Experience: Recent primary and community nursing experience; nurse-led management of minor illness, minor ailments and injuries; nurse-led triage; compiling protocols and clinical guidelines; clinical leadership; audit research.
  • Knowledge: Advanced clinical practice skills; management of patients with long‑term conditions; management of patients with complex needs; clinical examination skills; respiratory focus; accountability of own role and other roles in a nurse-led service; local and national health policy; clinical governance issues in primary care; patient group directions and associated policy; knowledge of public health issues; able to identify determinants of health in the area.
  • Qualifications: Maintain registration with the Nursing and Midwifery Council (NMC); work in accordance with the NMC Code of Professional Conduct; project management; working with community development initiatives; health needs assessments; experience of working within a Primary Care Network.
