Main duties of the job

The District Nurse / Senior Community Nurse is responsible for the assessment and management of the health needs of an identified population and caseload, focusing on public health, preventative care, and admission avoidance. They will support the team in implementation and evaluation of programmes of care within a range of target groups including preventative deterioration of complex health, health inequalities, social prescribing and signposting, management of comorbidities and supporting the prevention of an urgent care need.

The nurse will clinically supervise and provide support to regulated and unregulated team members with career development, clinical capabilities and behaviours, education and supervision. They will assist the Professional Lead with recruitment and retention, support the wellbeing of the team and improve quality outcomes and budget management as well as implementing service improvement schemes and supporting in pathway development.

Responsibilities and duties include:

  • Named responsibility and professional accountability for the management of a defined caseload including the delegation of tasks to other members of the team.
  • Provide visible leadership to ensure an equitable evidence-based service underpinned by clinical governance.
  • Prescribe within their individual scope of practice in line with V300 qualification and ensure nurse prescribing is undertaken safely and cost-effectively.
  • Develop a caseload profile and implement/evaluate programmes of care for target groups using a case management approach.
  • Act in accordance with local safeguarding and Trust guidelines to protect vulnerable adults and in line with the NMC Code of Professional Conduct.
  • Contribute to achieving Trust objectives and support the Professional Lead in change, quality outcomes and pathway development.
  • Provide nursing care to patients on defined caseloads and offer specialist advice to patients, carers, and other agencies.

Professional Development and Leadership

Participate in development, training and mentorship of pre-registration students/medical students and staff, including induction and preceptorship programmes. Attend meetings and deputise for the Specialist District Nurse and/or Professional Lead as required. Support the induction programme for new staff.

Communication

Demonstrate excellent communication and negotiation skills in complex or sensitive situations. Promote high standards of data protection, dignity and confidentiality. Ensure effective communication to support continuity of care across care settings.

Analysis and Judgement

Critically evaluate team leadership and project management in line with national and local policy. Maintain accurate record keeping, comply with data protection, manage resources and contribute to audits and research as appropriate.

Planning and Organisational Skills

Supervise and direct work of the team on a daily basis, input activity data on EMIS, and support a positive learning environment with supervision, mentorship and preceptorship.

Responsibility for Patient Care

Assess, plan, and evaluate evidence-based care to meet needs and prevent disease. Perform comprehensive assessments, plan and implement care (including prescribing) and escalate when required. Skills include:

  • End of Life Care – Syringe Drivers and symptom control
  • Intravenous Therapy
  • Management of Plural Drains
  • Wound Care including complex therapies
  • Leg Ulcers and compression therapy
  • Patient education and self-management training
  • Management of multi-morbidity and deterioration prevention

The District Nurse will work with the Specialist District Nurse / Professional Lead to achieve performance indicators and maintain excellent working relationships with stakeholders.

Policy and Service Development

Lead quality initiatives and contribute to service redesign and policy development. Interpret national frameworks for local implementation and engage in audits and policy implementation.

Human Resources and Professional Responsibilities

Manage and support band 5 nurses, participate in recruitment and retention, ensure mandatory training, conduct Performance Development Reviews, promote wellbeing, and uphold professional responsibilities and freedom to act. Promote clinical supervision and ongoing professional development.

Research & Development

Identify, apply and disseminate research findings; complete clinical audits; stay informed on current issues and promote best practice.

General

The post holder must have access to independent transport for work purposes and be able to travel across the Trust and to partner services. The Trust operates a smoke-free policy.

Person Specification

Qualifications

  • Registered Nurse (1st Level)
  • First level degree in related subject
  • Evidence of Continuous Professional Development relevant to practice
  • Evidence of leadership or management training
  • Practice Educator and Assessor Preparation (PEAP) or equivalent
  • Coaching CPD

Experience

  • Significant post-registration community experience
  • Post-registration experience of caseload management
  • Experience in personalised care planning and complex patient management
  • Experience in managing risk and complaints
  • Experience in service improvement

Skills

  • Understanding of governance, audit, and risk management
  • Advanced clinical decision-making and patient-centred care
  • Advanced clinical skills across conditions and comorbidities
