We are seeking a proactive, enthusiastic and dynamic Band 6 Nurse to join the Frailty team. The post holder will provide responsive support to patients over 75 years recently discharged from Warwick Hospital.

Responsibilities

  • Conduct telephone follow‑ups with patients, listening, addressing queries and clarifying issues around care.
  • Arrange home visits where required to support patients’ return to home life.
  • Collaborate with the Urgent Response team, CERT and the wider Community team as part of the Virtual Ward and MDT across wards.
  • Work with specialist colleagues in Respiratory, Heart Failure, Admiral Nurses, High‑Intensity User team and Falls services.
  • Contribute to project work and service development under the support of the team leader.
  • Support the Ward Manager in delivering a high quality, cost‑effective service and deputise when required.

Collaborations & Support

  • Full MDT approach including ACPs, Therapists and Geriatricians.
  • Operational and administrative support to facilitate care delivery.
