The predicted salary is between 37338 - 44962 £ per year.

As part of the new Neighbourhood Health Framework development, there is an excellent opportunity for a GP to support the transition into this new phase of working and develop their local leadership skills. The role will focus on specific portfolios covering across the three PCNs in South Tyneside. Currently, the main focus of the role will be Multimorbidity and the Secondary Care / Primary Care Interface although other portfolios may be added depending on need.

This position will work alongside the three existing PCN Clinical Directors, bringing a strong neighbourhood focus as services continue to evolve. The aim is to strengthen local integration and support the development of neighbourhood-based care. The post is offered on a 12-month fixed-term basis, allowing time to assess how the role integrates and functions operationally within the wider system.

Main duties of the job:

  • Lead on Multimorbidity and the Secondary Care / Primary Care Interface portfolio with respect to Neighbourhood Working from a PCN/Primary Care Perspective.
  • Liaise with various organisations and partners to facilitate collaborative working towards shared goals outlined in Neighbourhood Framework including the set up an integrated neighbourhood team, managing complex care proactively and improving the primary, community and secondary care interface.
  • Attend arranged meetings with Neighbourhood Partners.
  • Report back to PCNs the developments and updates as required.
  • Encourage engagement from PCNs to ensure voice is around Neighbourhood conversation tables.
