Wellbeing Team – Promoting Independence and Strength‑Based Practice
Be an advocate for independence, delivering holistic, strengths‑based support to adults in the community. You’ll work with individuals to review their needs and outcomes, promote wellbeing and independence, and empower individuals to live safely and well. Build strong local networks and become a trusted presence in people’s lives.
Responsibilities
- Work alongside adults to complete strengths‑based reviews, focusing on what matters most to them.
- Support people to maintain independence, improve wellbeing, and build on their existing strengths and networks.
- Work collaboratively with families, carers, and partner agencies.
- Promote community connections and access to local support.
You will play a vital role in helping people remain independent and live fulfilling lives in their own communities.
