We are looking for highly skilled, enthusiastic, dynamic self-motivated physiotherapists to join our Community Falls Team based at Washington Galleries.
Would you enjoy the challenge of adopting a MDT approach to providing assessment and interventions to reduce the risk of falls in the delivery of high-quality care for the patients in the community setting? If so, this could be the job for you. You will be responsible for providing care to people within the clinic setting, undertaking comprehensive holistic assessment of patients, prioritising clinical need, initiating investigations, determining a differential diagnosis, and implementing a plan of care reflecting evidence-based guidance in conjunction with NICE guidance.
Main duties of the job:
- Community Falls Team working hours 08.30-4.30 Monday - Friday.
- You will adopt an MDT approach and undertake triage within the service determining clinical priority of patients and assess, design, and deliver specialised interventions, exercise and education programmes for patients referred to the Falls service within the clinic setting.

Job responsibilities:
- To work as an autonomous practitioner within the community Falls Team using specialist clinical reasoning skills to provide specialist physiotherapy assessment, diagnosis, treatment and advice to an independently managed caseload which will incorporate routine and complex presentation of conditions.
- To begin to develop specialist skills to take a lead role in clinical practice issues, raising the profile of the service and offering specialist advice to planning and service development teams to assist in the implementation of service improvements.
- To consolidate post-graduate skills and continue professional development, including participation in physiotherapy in-service training programmes and external postgraduate training.
- To develop the clinical education of staff within the specialism and be responsible for the supervision of unqualified and newly qualified Physiotherapists who work under your direct supervision.
- Participate in research, service development and/or quality improvement activities.

Experience:
- Comprehensive experience at undergraduate and postgraduate level in Falls Assessment.
- Autonomous practitioner responsible for assessing, clinical decision making, treatment selection and evaluation of treatment and discharge.
- Evidence of effective verbal and written communication skills with complex patient presentations.
Qualifications:
- BSc Physiotherapy or equivalent.
- HCPC State Registration.
- Continuing Professional Development diary including evidence of reflective practice, training and clinical experience.
- Basic numeracy and literacy skills to level 1 or equivalent.
- Basic IT skills.
- Evidence of post-graduate competence, knowledge and skills in Falls Assessment.
- An understanding of South Tyneside and Sunderland Foundation Trust, its culture, strategic direction, achievements and NHS standing.
- A proactive ability to represent the profession and the Trust with positive enthusiasm.
- Commitment, motivation and adaptability.
- Good levels of general fitness.
