This is a full-time hybrid role for a Care Coordinator based in Manchester, with the flexibility to occasionally work from home. The Care Coordinator will be responsible for organizing and supervising care plans, maintaining regular contact with clients, managing case files, and ensuring high-quality care delivery. Daily tasks include liaising with healthcare professionals, responding to client inquiries, scheduling home care services, and ensuring client satisfaction through excellent customer support.
Qualifications:
- Care Coordination and Case Management experience, ensuring seamless service delivery and effective care plans
- Strong Communication skills to build and maintain relationships with clients, families, and healthcare professionals
- Experience in Home Care services with a focus on meeting the unique needs of individuals
- Customer Service experience with a commitment to providing excellent support and resolving concerns efficiently
- Ability to work both independently and collaboratively as part of a professional team
- Highly organized with attention to detail for managing client files and scheduling
- Proficiency in using scheduling and care management software is a plus
- Bachelor’s degree in Social Work, Healthcare Management, or a related field is preferred, but equivalent experience will be considered
