This is an exciting opportunity for an Advanced Nurse Practitioner to join South Lincoln Healthcare PCN in a temporary capacity as part of our exciting and innovative Home Health Team. This role is for 15 hours per week, on a 12 month fixed term contract.

The post holder will use specialist knowledge and skills to provide healthcare autonomously, being accountable for independent clinical assessment, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions. They will practice at an advanced level, demonstrating in-depth knowledge and competence in all aspects of frailty, encompassing advanced clinical assessment, critical thinking and clinical management skills that are evidence based and deliver high quality patient centred care.

The post holder will work within and across designated care homes for patients registered at our 7 member practices along with undertaking work for the PCN. The post holder may also be expected to work within other areas of the Primary Care Network. The role will support the PCN's passion for providing an all-encompassing approach to personalised care and health inequalities across the PCN. The post holder will need to hold a full driving licence and be prepared to travel between locations to support the remit of the role.

Main duties of the job

  • The role will work as a core member of the multi-disciplinary Home Health team.
  • The post-holder will manage the complete clinical care of patients, including planning and delivering treatments.
  • Assess and triage patients, and as appropriate provide definitive treatment.
  • Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.
  • Use complex decision making to inform the diagnosis, investigation and complete management of episodes of care within a broad scope of practice.
  • The role will require the ability to work flexibly to meet the needs of the organisation.

Job responsibilities

  • Assess, diagnose and treat care home patients with complex and undifferentiated health conditions, acting independently within professional guidelines.
  • Manage the complete clinical care of patients, including planning and delivering treatments, making necessary referrals to other specialists, and facilitating continuity of care.
  • Work as a core member of the multi-disciplinary Home Health team, providing expert clinical advice to colleagues and other members of the healthcare team.
  • Request investigations such as blood, urine and other laboratory tests, electrocardiographs (ECGs), ultrasound scans and x-rays in accordance with IR(ME)R regulations and local policy.
  • Use high-level communication skills to explain complex medical information and sensitive issues to patients, their families and other healthcare professionals.
  • Provide additional capacity support for the PCN within our member practices and support day to day access and capacity.
  • Continuously re-evaluate findings and clinical response to treatment and establish an ongoing management plan as appropriate.
  • Prioritise health problems and intervene appropriately in complex, urgent and emergency situations, including initiation and leadership of resuscitation.
  • Complete the relevant training in order to provide multi-professional, clinical practice and CPD supervision to other roles within Primary Care.
  • Undertake clinical audit and quality improvement work across the PCN.
  • Be responsible for the day-to-day planning of personal workloads.
  • Follow organisational policies, protocols, procedures, and standards.
  • Report any incidents, accidents or complaints in a timely manner, taking appropriate action where necessary.
  • Develop themselves and the role through participation in clinical supervision, training and service redesign activities.
  • Contribute to a patient safety culture through reporting and investigation of incidents and undertaking proactive measures to improve patient safety.
  • Maintain accurate clinical records of all patient consultations and related work.
  • Support in the delivery of enhanced services and other service requirements on behalf of the PCN.
  • Participate in the management of patient complaints when requested to do so.
  • Undertake all mandatory training and induction programmes.
  • Attend a formal appraisal with their manager at least every 12 months.

Person Specification

Experience

  • Have at least 1 years advanced clinical practice experience in a primary care or urgent care setting.
  • Be working at masters level or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research.
  • Highly developed specialist knowledge which is underpinned by theory and experience.
  • Experience of dealing with vulnerable patients.
  • Prior experience working within or closely with care homes, or with complex frail older people in a community or primary care setting.
  • Experience in palliative care and advanced care planning.
  • Experience of working in a multi-disciplinary team within a primary care environment.

Skills

  • Effectively demonstrate delivery of the 4 pillars of advanced practice through a portfolio.
  • Demonstrate an understanding of the legal, ethical and professional responsibilities and accountability with regards to the advanced level, autonomous practice.
  • Ability to support and manage patients including assessment, planning, evaluation and monitoring.
  • Ability to triage patient needs and manage a clinical caseload.
  • Ability to demonstrate effective written, communication, counselling and interpersonal skills.
  • Ability to work autonomously and within professional boundaries.
  • Possess a commitment to ethical decision making, professional responsibility, and a high standard of clinical care.
  • Participation in clinical audit/quality improvement work.
  • Effective time management (planning & organising).
