PCN Care Home/Primary Care Advanced Nurse Practitioner

This is an exciting opportunityfor an Advanced Nurse Practitioner to join South Lincoln Healthcare PCN in a temporary capacity aspart of our exciting and innovative HomeHealth Team.

This role is for 15 hours per week, on a 12 month fixed term contract.

The post holder will use specialist knowledge and skills toprovide healthcare autonomously, being accountable for independent clinicalassessment, diagnosis and treatment of patients with undiagnosed andundifferentiated conditions. They willpractice at an advanced level, demonstrating in-depth knowledge and competencein all aspects of frailty, encompassing advanced clinical assessment, criticalthinking and clinical management skills that are evidence based and deliverhigh quality patient centred care.

The post holder will work within and across designated carehomes for patients registered at our 7 member practices (Bassingham Surgery,Branston & Heighington Family Practice, Brant Road Surgery, Church WalkSurgery, Cliff Villages Medical Practice, The Heath Surgery, WashingboroughFamily Practice) along with undertaking work for the PCN. The post holder mayalso be expected to work within other areas of the Primary Care Network.

The role will support the PCNspassion for providing an all-encompassing approach to personalised care andhealth inequalities across the PCN.

The post holder will need to hold a full driving licence and be prepared to travel between locations to support the remit of the role.

Main duties of the job

The role will work as a core member of the multi-disciplinary HomeHealth team. The post-holder will manage the complete clinical care of patients, including planning and delivering treatments.

Assess and triage patients, and as appropriate provide definitive treatment.

Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.

Use complex decision making to inform the diagnosis, investigation and complete management of episodes of care within a broad scope of practice.

The role willrequire the ability to work flexibly to meet the needs of the organisation andthis may include working outside of core General Practice hours or within ourEnhanced Access service.

Job responsibilities

Assess, diagnose andtreat care home patients with complex and undifferentiated health conditions,acting independently within professional guidelines.

Manage the complete clinical care of patients, including planning and deliveringtreatments, making necessary referrals to other specialists, and facilitatingcontinuity of care

Work as a core member of the multi-disciplinary HomeHealth team, providing expert clinical advice to colleagues and other members of the healthcare team.

Request investigationssuch as blood, urine and other laboratory tests, electrocardiographs (ECGs),ultrasound scans and x-rays in accordance with IR(ME)R regulations and localpolicy.

Use high-level communication skills to explain complex medical information and sensitive issues to patients, their families and other healthcare professionals.

Provide additional capacity support for the PCN within our member practices and support day to day access and capacity.

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the Primary Care team.

Manage undifferentiatedundiagnosed conditions and identify red flags and underlying serious pathologyand take appropriate action.

Use complex decision making to inform the diagnosis, investigation and complete management of episodes of care within a broad scope of practice.

Actively take a personalised care approach to enable shared decision making with the patient, their family or carers.

Continuously re-evaluate findings and clinical response to treatment and establish an on-going management plan as appropriate.

Prioritise health problems and intervene appropriately in complex, urgent and emergency situations, including initiation and leadership of resuscitation.

Complete the relevant training in order to provide multi-professional, clinical practice and CPD supervision to other roles within Primary Care, for example, first contact practitioners and personalised care roles.

Undertake clinical audit and quality improvement work across the PCN.

Be responsible for the day-to-day planning of personal workloads.

Follow organisational policies, protocols, procedures, and standards.

Report any incidents, accidents or complaints in a timely manner, taking appropriate action where necessary.

Develop themselves and the role through participation in clinical supervision, training and service redesign activities.

Contribute to a patient safety culture throughreporting and investigation of incidents and undertaking proactive measures toimprove patient safety.

Maintainaccurate clinical records of all patient consultations and related work.

Supportin the delivery of enhanced services and other service requirements on behalfof the PCN.

Participatein the management of patient complaints when requested to do so and participatesin the identification of any necessary learning brought about through clinicalincidents and near-miss events.

Undertakeall mandatory training and induction programmes.

Attenda formal appraisal with their manager at least every 12 months. Once aperformance/training objective has been set, progress will be reviewed on aregular basis so that new objectives can be agreed.

Performother general tasks as assigned.

Person Specification

Experience

• Have at least 1 years advanced clinical practice experience in a primary care or urgent care setting to evidence autonomous clinical practice
• Be working at masters level or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competencies
• Highly developed specialist knowledge which is underpinned by theory and experience
• Experience of dealing with vulnerable patients
• Has a personalised care and What Matters to Me approach embedded within working practices
• Ability to work under pressure and manage varied workloads with changing demands
• Prior experience working within or closely with care homes, or with complex frail older people in a community or primary care setting.
• Experience in palliative care and advanced care planning
• Experience of work using telephone/remote triage
• Experience of working in a multi-disciplinary team within a primary care environment
• Knowledge of local primary care services

Skills

• NMC registered non-medical prescriber
• MSc Advanced Clinical Practice
• Evidence of continued professional development
• Graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (Supported) Route; both pathways enable eligibility for an Advanced digital badge issued by the Centre for Advancing Practice, demonstrating recognised educational and experiential preparation in advanced practice
• or
• Currently enrolled as a trainee on a Centre for Advancing Practice accredited MSc advanced practice programme
• or
• Currently enrolled on and progressed past the initial Learning Needs Analysis (LNA) stage of the Centre for Advancing Practice ePortfolio (supported) Route
