This is an exciting opportunity for an Advanced Clinical Practitioner to join South Lincoln Healthcare PCN as part of our expanding clinical team. The post holder will work within and across our 7 member practices (Bassingham Surgery, Branston & Heighington Family Practice, Brant Road Surgery, Church Walk Surgery, Cliff Villages Medical Practice, The Heath Surgery, Washingborough Family Practice) along with undertaking work for the PCN. It is anticipated that the post holder will mainly work across 3-4 practices, with a need to cover other practices on occasion, depending on need. The role will support the PCNs passion for providing an all-encompassing approach to personalised care and health inequalities across the PCN. Part time/job share would be considered. The post holder will need to hold a full driving licence and be prepared to travel between locations to support the remit of the role. It is anticipated that interviews for this role will take place on the 28/29 January 2026.

Main duties of the job

  • The post holder will be an integral part of both the PCN and General Practice Teams, and will work alongside GPs, Practice Nurses, HCAs, GPAs, Clinical Pharmacists, Pharmacy Technicians, Occupational Therapists, Social Prescribing Link Workers, Health & Wellbeing Coaches, Care Coordinators, Mental Health Practitioners and Musculoskeletal Practitioners.
  • The role will mainly consist of working within our member practices to enhance and support day to day access and capacity by providing a mixture of on the day and routine consultations which may be face to face, telephone, online or home visits. Time will be allocated for associated administration.
  • In addition, the post holder will spend a smaller proportion of their time supporting the PCN by working with our care home team. This would involve supporting with triage, telephone consultations and visits to care homes.
  • The role will require the ability to work flexibly to meet the needs of the organisation and this may include working outside of core General Practice hours or within our Enhanced Access service.

Job responsibilities

  • Integrate and work within our member practices to enhance and support day to day access and capacity.
  • Provide additional capacity support for the PCN care home team.
  • Assess and triage patients of all ages (including children), including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways or make necessary referrals to other members of the Primary Care team.
  • Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.
  • Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
  • Actively take a personalised care approach to enable shared decision making with the presenting person.
  • Complete the relevant training in order to provide multi-professional, clinical practice and CPD supervision to other roles within Primary Care, for example, first contact practitioners and personalised care roles.
  • Undertake clinical audit and quality improvement work across the PCN.
  • Optional for the right candidate: Provide programme leadership for our Cancer and/or Cardiovascular Disease Programme; Provide leadership for practice nursing teams (i.e. the importance of SMI health checks, elderly health checks, LD health checks); Provide support with the PCNs research programme.
  • Be responsible for the day-to-day planning of personal workloads.
  • Follow organisational policies, protocols, procedures, and standards.
  • Report any incidents, accidents or complaints in a timely manner, taking appropriate action where necessary.
  • Develop themself and the role through participation in clinical supervision, training and service redesign activities.
  • Contribute to a patient safety culture through reporting and investigation of incidents and undertaking proactive measures to improve patient safety.
  • Maintain accurate clinical records of all patient consultations and related work.
  • Support in the delivery of enhanced services and other service requirements on behalf of the PCN.
  • Participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through clinical incidents and near-miss events.
  • Undertake all mandatory training and induction programmes.
  • Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed.
  • Perform other general tasks as assigned. The list is an outline of the key tasks and responsibilities and is not intended to be an exhaustive list. The post holder will be expected to carry out other tasks, which may be reasonably required by the employer. The post may be subject to change/review as the needs of the role change.

Person Specification

Experience

  • Have at least 1 years advanced clinical practice experience, of which at least 6 months in a primary care or urgent treatment centre (or similar) setting, including managing a full range of undifferentiated undiagnosed conditions in patients of all ages (including children), to evidence autonomous clinical practice.
  • Be working at masters level or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competencies.
  • Highly developed specialist knowledge which is underpinned by theory and experience.
  • Experience of dealing with vulnerable patients.
  • Has a personalised care and What Matters to Me approach embedded within working practices.
  • Ability to work under pressure and manage varied workloads with changing demands.
  • Experience seeing patients of all ages, including children.
  • Experience of working in a multi-disciplinary team within a primary care environment.
  • Knowledge of local primary care services.

  • Ability to support and manage patients including assessment, planning, evaluation and monitoring, including signposting and provision of health promotion and education to patients and carers.
  • Ability to work autonomously and within professional boundaries, including when ordering and interpreting investigations and developing interventions.

  • Hold a current professional body registration.
  • Registered non-medical prescriber.
  • One of the following must apply to Masters level study:
  • Graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (Supported) Route; both pathways enable eligibility for an 'Advanced' digital badge issued by the Centre for Advancing Practice, demonstrating recognised educational and experiential preparation in advanced practice.
  • Currently enrolled as a trainee on a Centre for Advancing Practice accredited MSc advanced practice programme.
  • Currently enrolled on and progressed past the initial Learning Needs Analysis (LNA) stage of the Centre for Advancing Practice ePortfolio (supported) Route.
  • Evidence of continued professional development.
