An opportunity has arisen for a highly motivated individual to join our Adult Service Team. You will be working with Adults aged 16 years and older who will have a range of health and social needs. You will be responsible for the completion of a holistic assessment of risks and needs, developing care support plans to meet identified needs, reviewing existing support plans and supporting duty work under the supervision of the Team Leader. This work is underpinned by the principles of Self Directed Support legislation. The successful candidate will be motivated, organised, willing to learn and have experience of working with vulnerable groups in the community. We are recruiting for a Social Worker post within Adult and Older People\’s Services to provide a professional service to our client groups. We would welcome applications from registered Social Workers and from Final Year Degree in Social Work Students looking to start their career as a Social Worker. Final year students must be qualified within 2 months of submitting their application form.
