In partnership with community planning partners, voluntary and independent provider organisations, we offer a range of services designed to enable, support, improve and protect the health and social care of people using our services. As one of the largest services provided by the Council and the South Lanarkshire Health and Social Care Partnership, the Care at Home service plays a critical role in enabling individuals to remain at home in a way that promotes their wellbeing, dignity, privacy, respect and personal choices.
South Lanarkshire Health and Social Care Partnership alongside its commissioned Care and Home partners provide Care at Home services to over 3,000 service users every day. The service is continuing an exciting programme of transformation. To support this work, we are looking to appoint a Social Work Assistant.
As part of the Care at Home team, you will be responsible for ensuring the Council meets its statutory obligations. Timely assessment and review of service users serves to mitigate risks to service users and ensures that their health and wellbeing needs are met and the services that they receive are appropriate to their needs.
