The Hospital Discharge Social Work Team have a new opening for Social Worker. This is an exciting opportunity for experienced Social Work Practitioners who may wish to further develop their Social Work knowledge and practice within the University Health and Social Care Partnership.

Role and responsibilities include working with Adults and Older People requiring assistance with support planning to discharge from hospital. You will be working within the Social Work Hospital Discharge Team and collaboratively with Supported People, their families and Carers, and the wider Multi-Disciplinary Team within the acute Hospital setting and within the community setting where complex discharge from hospital requires reviewed.

The role involves gathering information for and completion of assessments, and practitioners require to be skilled and competent in relation to recognising, understanding, assessing, and managing risk related to working with vulnerable adults. Work further encompasses Adult Support and Protection and Adults with Incapacity legislative duties, training available where required, attending MDTs and facilitating discharge referrals.
