We have an exciting opportunity for a hardworking and enthusiastic individual to join our Assessment Management Team as an Occupational Therapy Assistant within Blantyre LIFE Intermediate Care.

About the Service

Blantyre LIFE Intermediate Care is a 24‑hour, short‑stay service delivered by South Lanarkshire University Health and Social Care Service. It is designed to reflect a home environment and operates using a reablement model of care. All service users have experienced a hospital admission and are medically fit for discharge but require additional support before safely returning home.

Responsibilities

  • Support service users to regain independence through Occupational Therapy principles, enabling safe returns home.
  • Work closely with individuals within the Blantyre LIFE Intermediate Care Unit, undertaking home visits and facilitating home discharges.
  • Provide reablement support around activities of daily living, including assessment, practice and evaluation of functional abilities.
  • Support improvements in independence and confidence.
  • Identify and recommend appropriate equipment where required.
  • Make onward referrals to statutory or voluntary agencies to meet identified needs.
  • Demonstrate the ability to work autonomously, manage tight timescales and take responsibility for own learning.
