Overview
SWRK 2530 – Social Worker (State Hospital) (605709) – South Lanarkshire Council. The State Hospital is one of the four high secure hospitals in the UK and has a remit to provide services to individuals across Scotland and Northern Ireland. The role involves engaging with high risk and/or high profile mentally disordered offenders, their families and carers. As a social worker you will complete a broad range of generic and specialist risk assessments, reviews, applications and reports. Operating as a social worker within a high secure hospital environment you will work across care groups to undertake and participate in multi-disciplinary assessments and care management processes, providing integrated care and treatment plans (inclusive of rehabilitation services) for individuals with mental health problems and their families/carers within the appropriate legislative framework.
Working within a tertiary setting and in partnership with other agencies/resources requires a commitment to multi-disciplinary working and the promotion of positive working relationships. You will work with a range of other disciplines and agencies to provide/contribute to ongoing care management processes for our patients, their families and carers, including risk assessments, risk management plans, CPA reports, outcome planning, and participation in case conferences/discussions within policy and legislative frameworks. You will undertake duties and planning activities to manage a caseload with accurate records, accountability, workload management, case recording, data collection and submission of reports in accordance with South Lanarkshire policies, procedures and standards, including the use of IT systems. You will undertake mandatory NHS training related to site security, personal safety, health and safety and hospital policies to ensure adherence to protocols.
Responsibilities
- Undertake a broad range of generic and specialist risk assessments, reviews, applications and reports for high risk and high profile Mentally Disordered Offenders.
- Conduct Admission Assessments, Appropriate Adult Assessments, Carer Assessments, Child Contact Assessments/Reviews (including non-face to face contact).
- Prepare Child Protection Reports/Risk Assessments and liaise with Local Authorities and other bodies; CPA Review Reports; MAPPA Transfer/Discharge Reports; Parole Reports; Pre-Admission Assessments; Suspension of Detention Reports.
- Contribute to case conferences/discussions within policy and legislative frameworks and produce CPA and other required documentation.
- Maintain accurate records, data collection and submission of reports per South Lanarkshire policies; manage workload and ensure proper use of IT systems.
- Engage in multi-disciplinary working and collaborate with other agencies to deliver integrated care and treatment plans and rehabilitation services.
- Adhere to health and safety, site security and NHS training requirements.
