Overview

An opportunity has arisen for a highly motivated individual to join our Hospital Discharge Social Work Team.

Location

Stonehouse Hospital, Larkhall

Hours

2 x 35 hrs or 3 x 17.5 hrs per week, temporary for 12 months

Work Pattern

2 week rolling rota across University Hospital Wishaw and University Hospital Hairmyers.

Salary / Hourly Rate

Full time: £38,779 – £39,910 per annum; Part time: £19,389 – £19,995 per annum.

  • Week 2 PT (1) – Mon, Tue

Responsibilities

  • Complete a holistic assessment of risks and needs.
  • Develop care support plans to meet identified needs.
  • Review existing plans and support duty work under the supervision of the Operations Manager.

Qualifications

  • Motivated, organised, and willing to learn.
  • Experience of working with vulnerable groups in the community.
  • Understanding of the Social Work (Scotland) Act 1968 principles.
