How You'll Make a Difference

As a Senior Commissioning Officer within the Partnerships and Commissioning team, you will play a pivotal role in transforming services for carers. Together with our provider partners Carers Support, you will lead on a project to adopt the Three Conversations model (3Cs). You will enable Carers Support to fulfil their delegated statutory functions around delivering carer assessments under the Care Act, adopting the 3Cs approach and support them in recognising the value of their initial contacts and signposting activity and how this aligns with the model. To achieve this, you will be working in close partnership with the senior leadership team in Carers Support, carers with lived experience and the local authority.

What You Will Be Doing

  • The primary purpose of your role will be to co‑develop and co‑implement a project plan to embed the Three Conversations model, including but not limited to creating new key performance indicators to measure success.
  • Working closely with commissioning colleagues, carers and Carers Support, you will ensure smooth project delivery.
  • This will include supporting Carers Support with new ways of working, such as maximising technology‑based solutions delivered under the Accelerating Reform Fund, which will embed and align to the delivery of strategic goals set out in the South Gloucestershire Carers Strategy.
  • Using your knowledge and expert guidance, you will provide hands‑on advice and support to Carers Support, including facilitating huddles, reviewing processes, and facilitating change internally to better align practices with the Care Act.
  • Working collaboratively with Carers Support, carers and other stakeholders, you will co‑design services and carer pathways that reflect the value of lived experience and maximise on new projects and tools to improve carer outcomes, making recommendations and supporting different ways of working.
  • You will deliver quarterly updates at the Carers Advisory Partnership and where required, liaise with other local authorities and organisations implementing similar initiatives to embed valuable learning.
  • Where appropriate, you will bring in‑depth knowledge of the Care Act, specifically the rights of carers, alongside an working knowledge of the Three Conversations model in practice.

What We Need From You

We require you to have a minimum of three years social care experience, with proven practice in delivering carer assessments within a Three Conversation model and an in‑depth understanding of carers' rights and the ability to navigate complex statutory frameworks. You will have experience in leading projects and the confidence to provide guidance to senior leaders and enable operational teams to embrace change. Given the elements of the role, you will have the ability to build strong relationships with a diverse range of stakeholders, including those with lived experience. You must have strong time management and problem‑solving skills, with the ability to balance competing priorities to meet deadlines. Ideally, you will have the experience or knowledge of the process for direct payments for carers, including ongoing and/or one‑off payments.

What You Need To Know

Your salary is pro‑rata to hours worked. This is a six‑month limited‑term contract, ending June 2026. We are also offering this as an internal secondment opportunity, you should first seek your line manager's permission before applying.
