How you’ll make a difference

As a Transition Assistant, you will work with children and young people to promote their independence, with the aim of them being able to reach their full potential within their home and wider community. Some of the children and young people you work with may have special educational needs and disabilities (SEND).

Based in Kingswood, the team sits within South Gloucestershire’s 0-25 Disabilities service. They have a focus on early interventions and supporting key transitions with those referred to the service.

What you will be doing

  • You will provide individual support to a child or young adult using a person-centred approach, contributing to them being able to access services that are available in their local community.
  • Undertaking regular reviews, risk assessments and visits to service users, you will offer advice and support ensuring that care meets the required standards.
  • It will be your responsibility to support the individual with travel requirements or be involved in a school travel training project, ensuring the service of fleet vehicles meet the required workplace protocols and vehicle checks.
  • Regularly, you will work with care groups and other agencies to share information and provide additional support.
  • It will be key for you to maintain confidential reports and case notes, as well as recording care plans and risk assessments in accordance with professional and council guidelines.
  • You will refer any issues in accordance with safeguarding procedures to protect vulnerable people.

What we need from you

  • We require you to hold an NVQ 2 or 3 GCSE’s (grade C/4 or above) or equivalent qualification or have relevant experience in office administration.
  • It is essential that you can demonstrate knowledge of relevant legislation, regulations and codes of practice, support mechanisms and specialist equipment.
  • You must have previous training and experience in a similar service environment.
  • Ideally, you will have experience in managing challenging behaviours, for example behavioural difficulties in homes or the community.
  • It would be beneficial to have some knowledge of other agencies, professional support groups and related external organisations.

What you need to know

  • You will be working Wednesday 12:30pm-4:30pm and Thursday and Friday 08:30am-4:30pm.
  • You will be expected to travel throughout the authority and surrounding area so you must have a full, valid driving licence with regular access to a vehicle or have an alternative means of travel.
  • Business insurance will need to be added to your current insurance policy.
  • Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) for both Children and Adults is obtained for this post.
