South Gloucestershire Council is seeking a Social Work Assistant to support disabled children and young people, their families and networks. You will work in the 16-25 team within the 0-25 social care service to help clients reach their potential.

You will work in a strength-based model (3 Conversations) and coordinate with the Pathway to Independence team, liaising with community, volunteers and external contacts to broaden support and promote independence.
