Where You'll Be Working
CAPAC (Community Acute/Post Acute Care) is a dynamic service that incorporates 4 essential programs; Hospital in the Home, Rehabilitation in the Home, Transitional Aged Care Program, and Healthy at Home. The service is a holistic multidisciplinary team that provides the right care for patients in the community to support them either to recover at home or to avoid an unnecessary hospital or residential aged care facility admission.
About The Role
TACP provides a multidisciplinary in-home rehabilitation program to clients after an acute hospital admission. The team consists of nursing, physiotherapy, occupational therapy, podiatry, dietetics, and social work combined with a full complement of community support services. As part of the nursing team, you will have an array of roles, from assisting the client with their personal care, attending clinical observations within the scope of practice, ensuring the client is able to attend their ADL management, and assisting clients in completing their home exercise program. You will work as a valued member of the multi-disciplinary team with the aim of increasing and maximising our client’s level of independence. This role reports to the Transitional Aged Care Program NUM and a full orientation will be provided on commencement.

- AIN qualification - preferably with rehabilitation experience.
- Confidence to work independently while knowing when to seek support.
- Ability to work within scope and provide quality care in a home setting.
